General Information

• Fasting is not required
• Female Clients who are pregnant and wish to proceed with X-Ray Chest will be required to sign a Consent Form, and the X-ray will be done with a protective Double Lead Shield
• Female Clients who are pregnant & DO NOT WISH to do X-ray chest can opt for Sputum exam for MTb.
• The whole procedure may take 2-3 hrs including issuing the certificate.
• Certificate will be issued to the candidate only(self collection for Above 18 Years)
• Please bring your payment receipt to collect the certificate.
• Note: It is mandatory to bring with you all old and recent medical reports, prescriptions, X-rays, with reports and certificates. Especially if you have a history of TB or cancer or any other major illness.
• Note: Clients may be requested to undress upto their underclothes during physical exam.

ID Proof

• Original passport (Preferred) OR Original updated Aadhar Card
• Copy of passport (1st and last pages) / Copy of Aadhar (If Presenting Original Aadhar Card)
• Original Birth Certificate of babies with a copy, if Passport or Aadhar card is not made
• 2 recent photos
• Note: Please write in capital letters your Full Name, Residential Address, Contact Number, and Personal Email Address on the back of the passport copy of each applicant, to avoid delays

• Work Visa: Company ID card and its copy or copy of the offer letter or sponsorship letter.
• Student Visa: Copy of the offer letter from the college/university.
• PR Visa and their dependants ITA and IME Form.
• Medical Examination Instructions Letter and the IME Form (IMM1017E) from IRCC.
• Please bring a hard copy of your COVID 19 Vaccination Certificate with ALL dates of your vaccines
• Below 18, years Immunization (Childhood vaccine) record details printout if available.
• Above 18, years Immunization (Childhood vaccine) record details printout if available. Adult Vaccines are available at the center.
• Please Print and bring ( Medical History Sheet) duly filled
• Note: It is mandatory to bring with you all old and recent medical reports, prescriptions (including for eyes), X-rays, with reports and certificates. Especially if you have a history of TB or cancer or any other major illness.

Rs 4000/- (11 - 15 yrs) (Urine test, Chest X-ray, Opthal & Physical check up)
Rs 3200/- (5 -11 yrs) (Urine test, Opthal & Physical check up)
Rs 3000/- (below 5 years) (Physical check up)

Charges will be extra for additional test requirement

• You will receive Acknowledgement letter (Information Sheet) within 2 hours after medical exam.(self collection) with payment receipt
• Fasting is not required
• Only candidates are allowed in the OPD, However If the candidate is below 18, one parent must accompany, if the candidate is above 75yrs, then one relative is allowed
• Female Clients who are pregnant and wish to proceed with X-Ray Chest will be required to sign a Consent Form, and the X-ray will be done with a protective Double Lead Shield
• Female Clients who are pregnant and do not wish to proceed with X-Ray Chest will be issued a Deferral letter and the case will go on HOLD.
• Note: Clients may be requested to undress upto their underclothes during physical exam.

• Female Clients who are pregnant and wish to proceed with X-Ray Chest will be required to sign a Consent Form, and the X-ray will be done with a protective Double Lead Shield
• However if they do not wish to do an X-ray during pregnancy the X-ray exam can be set aside, and may be completed after delivery.
• Note: Clients will be requested to undress upto their underclothes during physical exam.
